The Mental Health Integration Program (MHIP), supported and administered by the Community Health Plan of Washington in partnership with Public Health -Seattle and King County, integrates mental health screening and treatment in a collaborative care model (CoCM), including psychiatric case review and consultation. MHIP collaborative care teams in Washington State safety-net primary care settings serve diverse Medicaid and uninsured populations. Since MHIP’s inception in 2007, over 50,000 individuals have received integrated mental health services. In 2007 it began as a state-funded, two-county pilot for high risk uninsured adults in King and Pierce counties, but MHIP expanded statewide in early 2009 to include over 130 primary care clinics.
MHIP uses a patient registry (CMTS) to track and measure patient goals and clinical outcomes, and facilitate treatment adjustment if a patient is not improving as expected. MHIP also utilizes pay-for-performance mechanisms to support model fidelity and prioritize patient outcomes. Training and workforce efforts for this project focus on the whole team and all providers are trained on the fundamentals of CoCM.

Through Archstone Foundation’s Depression in Late Life Initiative, the Care Partners project seeks to improve depression care for older adults by building innovative and effective community partnerships. Specifically, the Care Partners project has the following goals: 1) develop late-life depression innovations among primary care, community-based organizations (CBOs) and family, 2) build a learning community of clinics, CBOs, and researchers in California who will work together on the Care Partners Late-Life Depression Initiative to improve care for depressed older adults, 3) conduct an evaluation of the developing models, and 4) develop and conduct a Learning Collaborative in Year 5 for California clinics and CBOs interested in improving depression care for older adults. Throughout the project, project teams at the University of Washington (UW) and UC-Davis (UCD) provide technical assistance and evaluation to support site development and sustainment. Together, the community-engaged partnerships have tremendous potential to improve access to care, patient engagement, patient care experience and quality of care. In addition, CBO and clinic partners are well primed to improve care through addressing the social determinants of health.

IMPROVING DEPRESSION CARE IN THE RURAL WEST: SOCIAL INNOVATION FUND

Poor mental health is a major public health issue that robs millions of people of their chance to lead healthy and productive lives. Depression alone doubles overall healthcare costs, worsens other medical conditions, and results in a staggering loss of productivity at work. In underserved rural areas in the WWAMI (Washington, Wyoming, Alaska, Montana and Idaho) region, a severe shortage of mental health providers compounds these problems.
Through a public-private partnership, the AIMS Center supported eight rural community primary care clinics serving low-income patients to implement collaborative care (also called collaborative care management or CoCM) in the WWAMI region, a geographic area served by the University of Washington School of Medicine and representing 27% of the land mass of the United States. These 8 clinics planned to serve 3,250 patients but ultimately enrolled 5,392 patients. This represents 16% of the total unique patients served by these clinics and is a significant increase of the patients they were able to reach before implementing CoCM.

The COMPASS (Care Of Mental, Physical And Substance-use Syndromes) initiative, funded by the Center for Medicare & Medicaid Innovation, evaluated the large-scale implementation of the TEAMcare model treating patients with depression and comorbid diabetes and/or cardiovascular disease. The initiative was implemented in 18 medical groups and 172 clinics across eight states. Participating clinics differed significantly in size, organizational structure, patient populations, and payment systems, thus demonstrating the feasibility of implementing the TEAMcare intervention in “real world” clinical settings.
The initiative involved 3,609 Medicare and Medicaid patients in eight states and is one of the largest collaborative care implementations to date. The results of the trial were published in a 2016 issue of General Hospital Psychiatry. Among patients with uncontrolled disease at enrollment, 40% achieved depression response or remission, 23% achieved glucose control and 58% achieved blood pressure control over the 11-month treatment period. There were large variations in outcomes across the medical groups, and rigorous implementation was associated with increased effectiveness. Researchers learned best practices for treating patients in primary care settings who have multiple chronic conditions, demonstrated the model can be effective for the target population when implemented well, and identified financial models that can sustain and scale a multi-condition collaborative care program.

Developed in 2015, the Collaborative Care Medicaid Program (CCMP) is a state-based program to help clinics fully integrate behavioral health screening and treatment into primary care settings and to provide reimbursement for those services. CCMP grew out of a New York State Department of Health grant-funded demonstration program called the New York State Collaborative Care Initiative, which ran from 2011 to 2014. Having demonstrated robust feasability and acceptability, along with improved clinical outcomes during the grant period, the New York State Office of Mental Health (OMH) was able to secure legislative funding for the creation of the CCMP. CCMP was the first Medicaid program in the country to provide reimbursement for Collaborative Care services for adult depression. Anxiety diagnoses (including PTSD) were later added to the Medicaid payment, as well as a lower age threshold of 12 years. Another unique feature to the Medicaid payment structure is the Quality Supplemental Payment (QSP) payment, which gives some clinics the opportunity to get an additional payment by achieving quality outcomes.
Now, over 200 clinics participate in CCMP, receiving free training and technical assitance from the AIMS Center and Concert Health, as well as discounted access to the AIMS Centers’ Care Management Tracking System registry and discounted training in Problem Solving Treatment therapy. Part of participation requires the submission of quarterly process and outcomes data to OMH, which is used to further improve the training and technical assitance offered to CCMP clinics.
Untreated mental health illnesses have serious consequences for families, but fewer than one in four depressed people who identify as mothers receive effective treatment. This project examined depression care and clinical outcomes for pregnant people and people who identify as either mothers or primary caregivers, treated in 14 clinics serving racially and ethnically diverse communities with lower incomes as part of the Mental Health Integration Program (MHIP). The outcome of this project was published in Family Practice. Huang H. et al (2012) found that although there was substantial depression improvement in all four of the ethnic groups studied (Asian, Black, Latinx, White), outcomes of Latinx patients were higher than those of Black patients regardless of other demographic or clinical factors. Notably, this study shows that more intensive care management in the first month of treatment for primary care can lead to better outcomes for pregnant people, and mothers or primary caregivers with lower incomes experiencing depression. Another study describes the experiences of care managers working in this program and found that motivational interviewing skills were a valuable asset in engaging patients in care, which generally leads to better outcomes.

In an effort to increase access to mental health care in Seattle and King County, the AIMS Center at the University of Washington partnered with UW Medicine to launch the Behavioral Health Integration Program (BHIP). BHIP uses collaborative care to bring mental health treatment into all of the UW Neighborhood Clinics, a system of twelve primary care clinics located throughout greater Seattle, as well as clinics at Harborview Medical Center and the General Internal Medicine clinic at UWMC Roosevelt. Like elsewhere, mental health is a big part of primary care in Seattle and King County; in 2009, 19% of all clinic patients had a mental health diagnosis. Although Collaborative Care has been implemented around the world, the AIMS Center and UW Medicine are very proud to be able to provide it in our own community.
BHIP utilizes a web-based Care Management Tracking System that supports population-based care, provides patient outcome measures, and assists in quality improvement efforts. In October 2012, several goals were established for the BHIP program: to increase patient access by care managers and across all BHIP clinics, to improve patient outcomes on measures of depression and anxiety, to increase provider satisfaction with care management, and to improve provider satisfaction with psychiatric consultation. When measured in August 2013, the BHIP program had exceeded initial targets for each of the seven indicators.
BHIP won a Psychiatric Services Achievement Award from the American Psychiatric Association in 2014, and a Washington Award of Excellence in Healthcare Quality from Qualis Health in 2016.
BHIP is our means of implementing collaborative care in the AIMS Center’s own community, and with support from the university it is a part of. It helps us foster local partnerships and develop support for and awareness of collaborative care.
The New York State Collaborative Care Initiative helped primary care residents learn how to effectively practice team-based care to treat mental health conditions, a skill that has become increasingly important as integrated care becomes more widespread. Outpatient clinics associated with teaching hospitals are implementing Collaborative Care around the state, increasing the quality of mental health care for thousands of New Yorkers. By providing different intensities of technical assistance, the AIMS Center evaluated what level of support was needed to effectively help organizations implement Collaborative Care. They helped set up a technical assistance team in New York to provide on-site assistance to six clinics. Twenty other hospital organziations received web-based technical assistance, including webinars and online tools. Organizations used a patient registry to track and measure patient goals and clinical outcomes, and facilitate treatment adjustment if a patient is not improving as expected.
DIAMOND was a collaborative effort of 9 health plans, 25 medical groups, and over 80 primary care clinics in Minnesota to implement and study Collaborative Care for depression. The AIMS Center provided consultation and coaching for the initiative. In addition, the National Institute of Mental Health (NIMH) awarded a $3 million grant to HealthPartners Research Foundation to study the DIAMOND initiative over five years. The study evaluated all aspects of DIAMOND, including effects of the program on patient satisfaction, productivity, and program cost-effectiveness. The project used a patient registry that tracks and measures patient goals and clinical outcomes, and facilitates treatment adjustment if a patient is not improving as expected.
The Alaska Mental Health Trust Authority provided pilot funding to a Federally Qualified Health Center in Anchorage to support implementation of Collaborative Care and two Alaska Native tribal health corporations to support implementation of Collaborative Care and SBIRT (Substance Brief Intervention and Referral to Treatment). The purpose of this program was to determine if integrated mental health care can be effective given the unique challenges faced by primary care clinics in Alaska.
The AIMS Center provided training and coaching to five primary care organizations in Texas to implement integrated care for the two mental health conditions most commonly encountered in primary care: depression and anxiety disorders.
A study evaluating outcomes of this program found that vastly different organizations were all able to integrate mental health into primary care settings that serve disadvantaged communities. All five organizations in this program showed meaningful improvement in patient depression outcomes, regardless of varying patient characteristics. Sites that achieved the best patient outcomes engaged patients early, with multiple care manager contacts in the first 3 months of treatment, and received consultation and supervision from psychiatric providers.

The New York State Office of Mental Health (OMH) asked Performing Provider Systems (PPSs) from all over the state that chose Collaborative Care for their Delivery System Reform Incentive Program (DSRIP) to nominate at least one of their clinics to participate in the Learning Network. Through a rigorous application process, 19 clinics were selected to participate. As a part of the Learning Network, these clinics will eventually be eligible to bill the monthly Medicaid case rate once they are trained and have the necessary staffing, infrastructure, and workflows in place to deliver effective Collaborative Care (CoCM).
In order to achieve these goals, OMH provided clinics with training, site visits, and access to tools that facilitate the implementation of Collaborative Care, including access to the AIMS Center’s Care Management Tracking System.
While Collaborative Care training and support provided by OMH is only available to these clinics for one year, OMH hopes that creating a network of clinics learning together will facilitate the success of Collaborative Care for clinics beyond additional training support. As part of the Learning Network, clinics are encouraged to build relationships with the other clinics through communicating and learning from one another along the way. Individual clinics are matched with similarly structured clinics to form several training cohorts to better facilitate learning. An experienced coach works with each training cohort throughout the implementation process. Regular calls with the training cohort will keep clinics connected and provide the opportunity to receive additional training support, discuss challenges, and learn what the other sites are doing.
The AIMS Center, NYS OMH, Qualis Health, and, most importantly, the training cohorts will provide clinics with the support and tools needed to ensure a successful CoCM program implementation. We hope that each clinic’s care team will continue to communicate with the other members of the learning network after the close of the year and the discontinuation of services.
The legacy terms “IMPACT Care” or “IMPACT Model” is largely synonymous with Collaborative Care. The terms originate from the IMPACT study, the first large randomized controlled trial of treatment for depression. The IMPACT study demonstrated that Collaborative Care more than doubled the effectiveness of depression treatment for older adults in primary care settings.
The Collaborative Care model (CoCM) is now recognized as effective in treating a wide range of behavioral health disorders - not just depression. A team-based Collaborative Care approach can also address anxiety and trauma disorders, chronic pain, substance use disorders including alcohol and opioids, and ADHD.
In the largest treatment trial for depression to date, a team of researchers led by Dr. Jürgen Unützer followed 1,801 depressed, older adults from 18 diverse primary care clinics across the United States for two years. The 18 participating clinics were associated with eight health care organizations in Washington, California, Texas, Indiana and North Carolina. The clinics included several Health Maintenance Organizations (HMOs), traditional fee-for-service clinics, an Independent Provider Association (IPA), an inner-city public health clinic, and two Veteran's Administration clinics.
Half of the patients were randomly assigned to receive the care normally available in their primary care clinic, including medications (70% of usual care patients) and/or referral to specialty mental health. The other half of patients were randomly assigned to receive the IMPACT model of depression care, also known as Collaborative Care. A patient registry was developed for the trial that tracked and measured patient goals and clinical outcomes and facilitated treatment adjustment if a patient was not improving as expected.
As reported in JAMA in 2002, Collaborative Care more than doubled the effectiveness of depression treatment for older adults in primary care settings. At 12 months, about half of the patients receiving Collaborative Care reported at least a 50 percent reduction in depressive symptoms, compared with only 19 percent of those in usual care. A handout of usual care versus IMPACT care presents some reasons why the results were as significant as they were. Analysis of data from the survey conducted one year after IMPACT resources were no longer available showed that the benefits of the IMPACT intervention persisted after one year. IMPACT patients experienced more than one hundred additional depression-free days over a two-year period than those treated in usual care.
Since the end of the trial, over one thousand organizations in the United States and internationally have adapted and implemented Collaborative Care with diverse patient populations and for a variety of behavioral health conditions.
