Collaborative Care Registry Design Considerations

CoCM registries vary widely in their sophistication, functionality, cost, and scalability. Options include:

  • A spreadsheet used alongside the EHR
  • The AIMS Caseload Tracker
  • A custom registry within the EHR or care management software system
