Collaborative Care engages the patient as an active participant in treatment. We’ve found that when the patient and their family are an integral part of the health care team, it increases motivation, empowerment, adherence, and satisfaction. Most importantly, involving patients in their own treatment has been largely associated with better health outcomes. Educating patients about symptoms and treatment is essential, as is involving them in developing a relapse prevention plan.

The fact that I can go to a check up visit to see my doctor and have her already know what my care manager and I discussed is so much easier than me trying to remember every thing.
