Collaborative Care engages the patient as an active participant in his/her treatment. We’ve found that when the patient and his or her family are an integral part of the health care team, it increases motivation, empowerment, adherence, and satisfaction. Most importantly, involving patients in their own treatment has been largely associated with better health outcomes. Educating patients about symptoms and treatment is essential as is involving them in developing a relapse prevention plan.
