Utilization of registry tools is a vital component to successfully implementing Collaborative Care. This webinar will include an overview of important considerations when choosing a population-based registry tool, followed by a panelist discussion, and Q&A. Panelist will highlight the process their organizations underwent to implement a population-based registry tool that facilitates Collaborative Care at a patient and program level.
