Impact of a national collaborative care initiative for patients with depression and diabetes or cardiovascular disease

An evaluation of the Care of Mental, Physical and Substance-Use Syndromes (COMPASS) initiative outcomes and impact. The evaluation found that rigorous implementation of the model was associated with better outcomes. Researchers also learned best practices for treating patients who are diagnosed with multiple chronic conditions and financial models that can sustain a multicondition collaborative care program.
