Bronx Behavioral Health Integration Project (Bronx B-HIP)

Montefiore Medical Center, the academic medical center and University Hospital for Albert Einstein College of Medicine in New York, has launched Bronx B-HIP to improve care for both pediatric and adult patients with a variety of behavioral health conditions through implementation of the Collaborative Care model in a number of its primary care practices. Montefiore is committed to treating a wide range of behavioral health conditions across the lifespan—newborn to geriatric—including depression, PTSD, panic disorder, GAD, alcohol use disorders, and ADHD (pediatrics). In addition to improving behavioral health of patients and the quality of their care, the project aims to promote ongoing program sustainability through a case-based payment model that pays providers to deliver a range of services and supports the requirements of the Collaborative Care model (e.g., face-to-face and non-face-to-face encounters). To implement the case-based payment model, Montefiore will work in collaboration with three health plans (Affinity, HealthFirst, and EmblemHealth). It will be the largest scale on which Collaborative Care has been tested at an academic medical center serving a safety-net population. The AIMS Center is providing a Care Management Tracking System [1] as well as educational training on a variety of topics.
