With our partners at the\u00a0<a href=\"https:\/\/aims.uw.edu\/nyscc\/the-aims-center\/\">AIMS Center<\/a>\u00a0providing national expertise, we are able to offer technical assistance and training to help practices successfully implement the model and be able to take advantage of the NYS CCMP reimbursement.<\/p>\n\n\n\n<p>Training&nbsp;includes implementation support and evaluation assistance to organizations planning and implementing patient-centered, evidence-based integrated care. The specific configurations of services is customized to meet the needs of each implementing organization or initiative.
