{"id":30,"date":"2024-07-19T14:03:32","date_gmt":"2024-07-19T14:03:32","guid":{"rendered":"https:\/\/aims.uw.edu\/nyscc-dev\/?page_id=30"},"modified":"2026-04-01T21:19:25","modified_gmt":"2026-04-01T21:19:25","slug":"eligibility","status":"publish","type":"page","link":"https:\/\/aims.uw.edu\/nyscc\/collaborative-care-medicaid-program-ccmp\/eligibility\/","title":{"rendered":"Eligibility"},"content":{"rendered":"\n<p>Primary Care practices that are interested in receiving Medicaid reimbursement for providing\u00a0<a href=\"https:\/\/aims.uw.edu\/nyscc\/about-collaborative-care\/\" data-type=\"page\" data-id=\"6\">collaborative care services<\/a>\u00a0must be approved by the New York State Office of Mental Health.\u00a0<a href=\"https:\/\/aims.uw.edu\/nyscc\/collaborative-care-medicaid-program-ccmp\/\" data-type=\"page\" data-id=\"23\">Collaborative Care Medicaid Program<\/a>\u00a0(CCMP)\u00a0reimbursement is open to physical health providers, serving those 12 years of age and older, including Pediatrics, Family Medicine, Internal Medicine and Women\u2019s Health. To qualify, practices must have the appropriate team members in place and adhere to the evidence-based principles of the Collaborative Care model. These include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"http:\/\/aims.uw.edu\/resource-library\/care-manager-role-and-job-description\">Behavioral Health Care Manager<\/a><\/li>\n\n\n\n<li>Designated Program Lead<\/li>\n\n\n\n<li>Data Manager<\/li>\n\n\n\n<li>Billing Lead<\/li>\n\n\n\n<li><a href=\"http:\/\/aims.uw.edu\/resource-library\/psychiatric-consultant-role-and-job-description\">Psychiatric Consultant<\/a>&nbsp;(MD\/PNP) and a minimum of 1 hour per week designated for consultation<\/li>\n\n\n\n<li><a href=\"https:\/\/aims.uw.edu\/nyscc\/training-opportunities\/cmts-registry\/\" data-type=\"page\" data-id=\"47\">Registry<\/a> to manage your Collaborative Care Caseload<\/li>\n\n\n\n<li>Standardized screening process using the PHQ-9<\/li>\n\n\n\n<li>Warm connection to the Behavioral Health Care Manager<\/li>\n\n\n\n<li>Utilize a treatment-to-target approach, using the PHQ-9 as a monitoring tool to identify patients that are not improving<\/li>\n<\/ul>\n\n\n\n<p>Please&nbsp;<a href=\"https:\/\/aims.uw.edu\/nyscc\/contact-us\/\" data-type=\"page\" data-id=\"17\">contact us<\/a>&nbsp;if you have any questions about any of the above readiness factors.
