Practices&nbsp;<a href=\"https:\/\/aims.uw.edu\/nyscc\/collaborative-care-medicaid-program-ccmp\/apply-to-participate-in-ccmp\/\" data-type=\"page\" data-id=\"36\" target=\"_blank\" rel=\"noreferrer noopener\">must apply to OMH<\/a>&nbsp;and be accepted into the Collaborative Care Medicaid Program (CCMP) before becoming eligible to use the Medicaid rates for reimbursement. If you have questions about if your site is eligible to use the rates, please contact&nbsp;<a href=\"mailto:NYSCollaborativeCare@omh.ny.gov\">NYSCollaborativeCare@omh.ny.gov<\/a>.&nbsp;<\/p>\n\n\n\n<p>Program participants must be able to demonstrate that they are following the evidence-based best practices of CoCM, and that they are able to report evaluative data to NYS OMH.
