Training Opportunities

For primary care practices that have not yet fully implemented the Collaborative Care model for integration but are interested in doing so, there are training resources available. With our partners at the AIMS Center providing national expertise, we are able to offer technical assistance and training to help practices successfully implement the model and be able to take advantage of the NYS CCMP reimbursement.

Training includes implementation support and evaluation assistance to organizations planning and implementing patient-centered, evidence-based integrated care.
