Feature:Do You Want to Implement Collaborative Care?

The New York State Office of Mental Health (OMH) offers primary care providers a unique opportunity to implement the Collaborative Care model (CoCM) and receive reimbursement for services provided. Practices must apply to OMH and be accepted into the Collaborative Care Medicaid Program (CCMP) before becoming eligible to use the medicaid rates for reimbursement.

Program participants must be able to demonstrate that they are following the evidence-based best practices of CoCM, and that they are able to report evaluative data to NYS OMH. Approved CCMP sites are eligible to bill the Collaborative Care case rate for any Medicaid patient receiving CoCM depression treatment.

Training is available to help practices implement the model to qualify for the reimbursement.
