Learning Network
In 2016, the New York State Office of Mental Health (OMH) commissioned Performing Provider Systems (PPSs) from all over New York State that chose IMPACT (collaborative care) for their Delivery System Reform Incentive Program (DSRIP) to select 19 clinics through a rigorous application process. As part of the first Learning Network, these clinics worked to establish the necessary staffing, infrastructure, and workflows to begin delivering effective collaborative care.
To achieve these goals, the AIMS Center, Qualis Health, and the New York State OMH provided clinics with collaborative care training, site visits, and access to tools that facilitate the implementation of collaborative care, including access to the AIMS Center’s Care Management Tracking System (CMTS).
Building upon the success of the first Learning Network as well as the lessons learned, the AIMS Center is once again partnering with the New York State OMH to uphold the Learning Network by providing training, technical assistance, implementation support and evaluation assistance to organizations planning and implementing patient-centered, evidence-based integrated care.
