Social Innovation Fund: Healthy Futures
Providing effective depression care in rural areas is extremely challenging given the lack of resources, high rates of poverty, prevalence of uninsured patients, and lack of mental health professionals. The Social Innovation Fund is helping primary care community clinics deliver better mental health care to approximately 8,000 adults by funding implementation of the IMPACT/Collaborative Care model in rural areas of the western United States.
Each of the eight clinics chosen to participate in the SIF initiative receive coaching from the AIMS Center during all phases of the implementation, from building a vision of how the model fits into their organizational mission to developing a plan for long-term financial sustainability after the grant ends. All members of the IMPACT/Collaborative Care team at each clinic receive in-person training, and ongoing training efforts use a combination of webinars and presenting cases to develop new care managers. Group phone calls enable care managers and psychiatric consultants from each implementation site discuss common concerns and educational topics key to sustaining Collaborative Care efforts. In addition to providing training and coaching assistance, the AIMS Center is testing the effectiveness of the model in terms of cost and clinical outcomes, something previously unresearched in rural settings. The implementation is using a patient registry (CMTS) to track and measure patient goals and clinical outcomes, and facilitate treatment adjustment if a patient is not improving as expected. Ultimately, we hope the project helps deliver better mental health care to rural populations around the world.
